2026 Health & Welfare Trends & Outlooks

Employee Benefits in 2026 "play a bigger role than ever in shaping the day-to-day employee experience,"1 keeping existing employees engaged and productive, while helping employers attract new talent.

According to one study, “almost three-quarters of American workers say benefits matter as much as—or more than—their pay."Similarly, when hiring new talent, Robert Half reports that "A 6-figure offer can get someone’s attention—but in 2026, it’s the perks and benefits that close the deal."3

But as numerous forces drive up the cost of employee benefits, HR must strike a balance between growing employee expectations for comprehensive, personalized, yet affordable benefits. Meanwhile, employers are increasingly challenged with economic and cultural sustainability, frequently being discussed at the C-suite level.

Amid these pressures, what trends and outlooks will shape 2026?

To help answer this question, we once again called on our trusted partners for their insights into 2026 and compiled this annual compendium of outlooks and trends. Below, NEEBC offers a synopsis of what we learned.

Across these 2026 industry outlooks, three forces stand out as the biggest drivers shaping the future of benefits:

  • rising health and pharma costs—nationally and globally—are influencing plan design and cost management strategies;

  • employees expect an improved and broader well-being experience; and

  • the workforce has a growing appetite for personalized benefits.

But the overall picture doesn't end with these three forces. The partner reports shared below spotlight many additional trends to considersome emerging like the future use of GLP-1s for conditions beyond weight loss and “virtual first” care; and some evolving like the growing role of agentic AI and predictive analytics and direct to primary care arrangements now allowed under HDHP/HSA plans.

We invite you to read these reports now and refer to them throughout the year as you make decisions, seek inspiration, and plan for 2027 and beyond.

Aon

This report finds that global medical costs are expected to rise 9.8% in 2026, remaining elevated but continuing a gradual deceleration from recent peak levels. Behind these increases are familiar pressures: higher utilization, aging populations, the rapid adoption of advanced medical technologies as well as the growing impact of high-cost prescription drugs like GLP‑1 therapies.

The report also highlights significant regional variation with North America, APAC, and parts of Latin America and the Middle East/Africa facing especially strong cost headwinds. In response to these trends, leading employers are doubling down on wellbeing initiatives, flexible benefits, and targeted cost-containment strategies to balance affordability with competitive and sustainable health benefits.
